Fentanyl Treatment Program: Levels of Care

Monima provides outpatient programs for individuals facing challenges with substance use, including both alcohol and drug-related concerns. Our Intensive Outpatient Program (IOP) and Partial Hospitalization Program (PHP) offer tailored levels of care to ensure each client receives the appropriate support. In addition, we provide continued recovery assistance through outpatient treatment and our alumni program.

The Difference Between IOP and PHP

The main difference between PHP and IOP lies in the weekly hours clients dedicate to therapy and interactions with our medical team. Both programs are thoughtfully structured to provide distinct levels of care that meet the diverse medical, emotional, physical, and psychological needs of each person.

Intensive Outpatient Addiction Treatment (IOP)

Monima’s Intensive Outpatient Program (IOP) is designed for women in San Diego seeking part-time treatment for moderate to severe alcohol or drug addiction. This structured outpatient program provides a higher level of support than standard weekly therapy and can serve as a bridge between full-time programs—like PHP or residential treatment—and resuming daily life. Participants in IOP engage in 9–15 hours of therapy each week.

Partial Hospitalization Program (PHP)

Monima’s Partial Hospitalization Program (PHP) in San Diego offers the highest level of outpatient care available. This structured program is ideal for individuals whose severe fentanyl addiction has started to disrupt daily life. Many clients temporarily step away from work, school, or other routines to fully focus on recovery. During PHP, participants take part in five to six hours of group and individual therapy each day, five days a week.

Outpatient Treatment

At Monima Wellness Center, we recognize that recovery continues beyond the completion of intensive treatment. Our outpatient program (OP) is designed to provide ongoing support for women in San Diego who have finished higher levels of care. This program ensures continued access to therapy, counseling, and other clinical services, giving clients the tools and resources needed to sustain progress and reduce the risk of relapse.

Alumni Program

At Monima Wellness Center, our alumni program fosters a supportive community for women beyond treatment, emphasizing connection, wellness, and ongoing personal growth. Monima has teamed up with Ohana Recovery Residences to offer optional safe housing for women participating in our outpatient fentanyl addiction programs. Residents at Ohana benefit from a supportive, immersive recovery setting and a community of like-minded women.

Somatic & Nervous System Healing

Addiction rehab, Intensive Outpatient Programs (IOP), and Partial Hospitalization Programs (PHP) differ mainly in how much structure, medical oversight, and time commitment each level of treatment provides. The right program depends on the severity of the addiction, mental health needs, and how much support a person requires to stabilize and recover.

Addiction rehab (often used to describe residential or inpatient treatment) offers the highest level of care. Clients live on-site and receive 24/7 support in a fully structured environment, making it ideal for individuals with severe addiction, unsafe home environments, or repeated relapses.

PHP (Partial Hospitalization Program) is a step down from inpatient rehab but still very intensive. Clients attend treatment most of the day, several days per week, while living off-site. PHP is appropriate for individuals who need close clinical oversight but do not require overnight care.

IOP (Intensive Outpatient Program) offers greater flexibility. Clients attend treatment for fewer hours per week and continue working, attending school, or caring for family. IOP works best for individuals with a stable living environment who are able to manage recovery with structured support rather than full-time supervision.
